Communicable Disease Report Card Instructions
The
Communicable Disease
staff of Clay County
Health Department is
responsible for the
prompt investigation
of disease outbreaks
and unusual situations
and to implement control
measures to minimize
further transmission
of disease. Communicable
Diseases are diseases
that can be spread from
person to person and
also from animals to
man. Many of these can
be controlled by appropriate
vaccines or treated
with medications.

Immunizations - Clay
County Health Department's
immunization program
provides all childhood
immunizations, Monday thru
Friday by appointment.
Certain immunizations
are required by NC
prior to children entering
public school systems.
Many colleges also
require updated immunizations
prior to entrance.
The
Health Department
utilizes the North Carolina
Immunization Registry
(NCIR). This is an electronic
system of record keeping
which is utilized state
wide by the health departments.
Upon completion of
data entry, the parent
may receive a printed
shot record with the
North Carolina State
Seal as a water mark.
Child
Immunizations (ages
0-18) are free of
charge. See the Immunization

Adult
Immunizations -
(ages 18 and up) are
also available by
appointment; fees
for these vary according
to the vaccine needed.

Hepatitis A Vaccine
Hepatitis B Vaccine
Twinrix (Hepatitis A & B
combined)
Influenza
(Flu) Vaccine –seasonally
Pneumonia Vaccine

Investigation
and Control of Disease
Outbreaks - Health
Care Providers such
as hospitals, doctor's
offices, labs, etc.
are required by North
Carolina law to report
to the Health Department
within specified time
frames certain communicable
diseases/conditions
. Others such as restaurants,
schools, and daycare
centers are also required
to report such diseases
or conditions.
The
Health Department
surveillance team then
initiates an investigation
into the situation to
identify people who
might have been exposed
and begin preventative
measures to reduce the
incidence and prevent
further spread of the
disease. Quick intervention
is critical to the
identification and control
of such outbreaks.
Clay
County Health Department
provides confidential
testing, treatment,
and contact follow-up
of sexually transmitted
diseases, hepatitis,
tuberculosis, and
other communicable
diseases.Who
should report cases?
- Physicians, hospitals
and laboratories must
report known or suspected
cases of certain diseases
in the county in which
they are located.
- Restaurants must report
cases of known or suspected
food borne illnesses
among customers or employees.
- School principals
and day-care workers
must report known or
suspected reportable
diseases of individuals
in the facility.
Some
examples of these may
be a report of pertussis
(whooping cough) by a
doctor's office, a report
of food poisoning from
a local restaurant, a
positive HIV test from
a local laboratory, or
perhaps a positive TB
skin test from a medical
provider. The Health Department
is committed to controlling
the spread of communicable
disease by providing investigation,
surveillance, and education
to protect the health
